Almost of this technology is at least 30-40 years old. It's been tried & tested in the West & Russia. One has to be really inept to have failures.

There is no country whose indigenous products dont fail.
And if you have never heard about such failures then either the products are not indigenous OR the failures are hidden from the public.

I am not sure about India, but in Pakistan we test each component, subcomponent, assembly and then put them together only when every component performs as per design parameters. It does not mean that final product is 100% ok the first time, but at least it does not fall from the sky. In this age, specially in case of missiles 98% is electronics and 2% physical systems. Success or failure depends on electronics performance, that can be tested in labs / on ground. Physical atrtributes can be modified / tweaked to enhance aerodynamic performance after real test, that is why we see minor design changes in Pakistani missiles. I hope this would help.
